Outcomes Reports
• Outcome Report Form - Online Form
  All Funded Agencies and Venture Grant Agencies are required to complete an Outcomes Report for each program funded as outlined in the Business Agreement signed by the Agency and the United Way. This is an Online Form.




Online Form Tip:
• 
  Prepare your Online Form in a Word Document FIRST. Then, when your report is complete, "copy and paste" each answer into the online form and submit. Unfortunately, there is no "Draft" Option. Many agencies who completed these forms in the past report that by completing the Word document first, they were able to take their time and complete the form easier and with more accuracy.
